If you qualify for the following insurance programs, we will accept new memberships. Be sure to provide your silver sneakers or renew active number to register.

If you qualify for the following insurance programs, we will accept new memberships. Be sure to provide your silver sneakers or renew active number to register. You may need to call your insurance to have them provide you with this number. The membership includes use of our sport courts, track and fitness center. If you wish to use to pool, hot tub or attend fitness classes, you will need to upgrade your membership for a $60 annual fee for Crestwood residents or $75 for non-residents.

Fees:

With Silver Sneakers or Renew Active ID number:

FREE for use of sport courts, track, and fitness center
$60 annually for Crestwood Residents for use of pool, hot tub and fitness classes
$75 annually for Non-Residents for use of pool, hot tub and fitness classes

Registration:

Once you have been provided your Silver Sneakers or Renew Active number by your insurance, you may come in person to sign up at the CRWC located at 5331 w 135th St in Crestwood. We will cross check your ID number to make sure it is eligible in our system. You will need to fill out our quick application form and once all is approved you'll be given your scan card and the membership will be complete.

CRWC Staff cannot sign you up for insurance programs or deal with your insurance in any way. You must already be apart of these programs to qualify for the benefits.
