Activity Registration Form
- Proof of Residency is required at the time of registration to receive resident rates.
- All memberships and programs must be paid in full at the time of registration.
- A parent/guardian must be present to sign a wavier form for anyone under the age of 18 years old.
- Seniors are anyone 65 years of age or older. Proof of age is required to receive the senior membership rate.
- No one under the age of 16 years old is permitted in the fitness center.
- No one under 18 years old is permitted in the whirlpool.
- No one under the age of 16 years old is permitted in the pool area without an adult unless participating in a supervised program.
